Documentation For A Smooth Appointment Process
To ensure that your appointment goes smoothly and that we have everything we need from you to provide your certification, please make sure to provide these at your appointment:
Currently, we only certify CLASS 2 and 3 pilots. Please make sure to visit the FAA forms site to complete the 8500 form prior to your appointment. Make sure you bring your Med-X-Press confirmation number to your visit.
Government or state-issued identification photo
A copy of your SODA or Special Issuance if applicable
Most recent medical record progress note from your physician for any medical conditions
Written or typed list of all medications and their dosage
If you wear glasses or contacts, please wear them for your exam

About The AME Exam
When you book your appointment with Wellness Restoration, you’ll find that the process is easy, efficient, and quick! Your exam will be completed during one visit and you will be able to receive your certification the same day, provided your exam is not deferred to the Regional Flight Surgeon. In addition to keeping you on track with same-day certification and one-day appointments, we’re proud to offer a $20 discount for all new FAA patients we see!
